                                                       Scholarship Application Check List/Instructions



1. Applicant must be a citizen of the United States and a graduating high school senior residing in Pinellas County. Include a copy of birth certificate, naturalization papers or information page of U.S. passport. Note: Temporarily cover the photograph if photocopying naturalization or passport papers. The applicant may or may not be a child of a DAR member.

2. Applicant must prepare a one-page statement of 1,000 words or less setting forth his/her career objectives, specifying how college major or college plans relate to future professional goals, and reasons for these choices.

3. Include an original copy of your high school transcript. Transcripts must indicate class rank/class size and test scores, unless home schooled. Home-schooled students include grade transcripts for grade 9 through the current year.

4. Include at least two but no more than four letters of recommendation from persons in authority from the high school the applicant attends, a minister or an employer who is familiar with the applicant's work. Letters may cover such things as the applicant's ability, work habits, integrity, character or potential. Each letter should not exceed one (1) page in length. Letters from a relative will not be accepted.

5. Include a list of extra-curricular activities, honors received, scholastic achievements, or other significant accomplishments. Maximum two pages.

6. Use this Check List/Instructions to make sure you have all necessary papers, signatures and four (4) copies of your entire scholarship package. Packages should not exceed fifteen pages.

7. The due date for receipt by Scholarship Committee is April 1, 2010.

NOTE: Winners will be notified by May 1, 2010.
